Home Visits

 If possible please try to telephone reception before 10 am if you require a home visit.

 You may only request a home visit if you are housebound or are too ill to visit the practice. A clinician will only visit you at home if they think that your medical condition requires it and they will make the decision as to how urgently a visit is needed. Our Care Navigators may ask you some routine questions so that they can give the clinician the information that is required ahead of the visit.

 A clinician may ask a community nurse to visit you at home.

 You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a the practice and have a child under five years.
